Millions in Funding for Alaska Rural Health Transformation

The Rural Health Transformation Program (RHTP) is one of the most significant healthcare investments in Alaska’s history: $272 million in Year 1 with up to $1.36 billion over five years. The objectives are to improve health care access and outcomes with sustainable care delivery models across Alaska’s vast and isolated geographic regions.

Organizations across Alaska are preparing Letters of Intent (LOIs) to be considered for this federal funding. The first LOI deadline is March 11, 2026.


The Alaska Community Foundation, facilitating grant administration on behalf of the Alaska Department of Health (DOH), is soliciting proposals for projects through Letters of Intent (LOIs). Organizations at varying levels of readiness may apply for funding through multiple pathways designed to match their current capacity and needs:

For organizations that want to make sure they’re fully set up to manage federal funding effectively.

For organizations with a strong idea for transforming rural care who need time to shape it into a sustainable, launch-ready project.

For projects that are ready for execution now that advance rural health transformation goals.

For projects identified by the DOH as addressing a high-impact need.
